Birth/Adoption
Within 90 days of the birth or adoption of a child:
*It is recommended that Health Plan changes be made within 30 days to avoid any delay in processing claims.
Health Plan (Medical/Dental) - Add child/children and spouse by electing family coverage option or add as dependents if you already have family coverage. Coverage effective on the date of birth or adoption. (Click here for Flexible Benefits Change Form)
Within 30 days after return to work:
Dependent Care Flex Account - Can enroll or increase annual election of Dependent Care flex-spending account. (Click here for Flexible Benefits Change Form)
Within 30 days of the birth or adoption of a child:
Life Insurance - Can obtain dependent life insurance for child/children without evidence of good health. Life/Disability Enrollment Form available in HR office.
Medical Flex Account - Can enroll or increase annual election of Medical flex-spending account. (Click here for Flexible Benefits Change Form)
Changes to consider at any time:
Beneficiary Changes - Update beneficiaries for Insurances and Retirement Annuities.
- Insurance Beneficiary Designation Form is available in HR office.
- Retirement Annuities changes can be done on-line at TIAA-CREF website.
- Voluntary Accident Insurance Plan Form is available in HR office.Update Dependents - If you are a participant in the Emeriti Health Account, call the Emeriti Service Center at 1-866-363-7484.
Tax (W-4) - Adjust W-4 to make changes to number of tax allowances if desired. Must complete W-4 form and submit to Payroll (Click here for form).
In Preparation for Your Leave:
